How we saved this precious baby when the mothers membranes ruptured prematurely?
Mrs R was a 28 yr teacher from Trichy. She was unsuccessfully trying for a pregnancy for 4 years after her marriage. Multiple medications by her local doctors had failed and pushed her into a state of desperation and depression. She visited Dr Priya on hearing from one of her friends and she was immediately comforted by her kind demeanor and empathy. Further tests indicated that she had polycystic ovarian disease with irregular cycles. A course of medications to regularize the cycles, life style modifications followed by ovulation induction was sucky and with Hods grace Mrs R did successfully conceive a twin pregnancy.She went back to her home town and was progressing well till 21 weeks. Then one fine day she developed rupture of her membranes with breaking of waters. The family naturally very worried took her immediately to local Gynecologist who diagnosed it as premature rupture of membranes(PROM) and also the shocking news of demise of one of the twins due to the leak. She was advised to terminate the pregnancy as the other 21 week twin was too premature to await pregnancy continuation till 40 weeks. There was a very high risk of the fetid and the mother developing an infection and endanger the lives of both. The baby was too precious a baby conceived after 4 long years to lose. The patient immediately rushed to us and after thoroughly evaluating the patient and understanding the circumstances offered to help her try and save the child. In fact other consultants in the same hospital opined that saving the 21 week fetid with ruptured membranes was never tried there before and were doubtful of its success. We did plan and put the patient on medications to weaken uterine contractions, antibiotics and special progesterone hormones to prevent pre term labor. Extreme care was taken to prevent ascending infection from the urinary tract and bowels. Every single day passing by was an achievement of sorts that helped the fetus grow in the mother’s womb. With fervent prayers the mother was successfully nursed till end of the 28 weeks and a beautiful 900 gm girl child was delivered by a Caesarian section . The preterm infant required next 3 weeks of neonatal icu stay and recovered rapidly and gained weight well before getting discharged from the hospital. 4 years later she is now a bubbly cute and adorable child already attending her play school. The family is ever grateful for Dr Priya's belief and determination in saving this precious child.
